And one thing worth stating plainly: the fee does not buy a diagnosis. You are paying for an expert opinion, and sometimes that opinion is that you do not have ADHD.

The Cost People Miss: Titration

If you are diagnosed and you choose medication, the assessment fee turns out to be the smaller number.

ADHD medication is started low and increased in steps until it works without unacceptable side effects. That process is called titration. NICE guidance is clear that it must be supervised by a clinician with ADHD expertise, with regular review and physical health monitoring throughout.

Medication and the Handover to Your GP

Once your dose is stable, the aim is usually to move prescribing to your GP under a shared care agreement. That is cheaper for you and it is how NICE expects the pathway to work.

Your GP is not obliged to accept shared care, and some decline. If that happens you can continue privately, which means an ongoing monthly prescribing fee rather than an NHS prescription charge.

What If You Are Not Diagnosed?

You still receive the full assessment and the written report, and the fee still applies. A report that says you do not meet the criteria is a clinical finding, not a failed purchase, and it often identifies what is going on instead.

Any clinic that implies a diagnosis is guaranteed is telling you something about itself.

Questions to Ask Before You Pay

Before booking anywhere, ask what the fee includes, who conducts the appointment and what their registration is, how long it lasts, whether the report and GP letter are included, what titration costs per month, and what happens if you are not diagnosed.

If a clinic cannot answer those six questions in writing, that is useful information.
